Monday 3/1/2010 ... afternoon update
Brandon had his embolization this morning and is now back in the ICU. It is hard to assess how successful the embolization was, but the doctor believes it went well and that Brandon will be ready for surgery tomorrow. The embolization took more than 2 hours and the waiting is the most horrible part. This procedure was a higher risk than the angiogram that he had a few days ago and had risks of bleeding and stroke during and after the procedure. The procedure involved inserting a tube into the artery in his groin and feeding that up to the arter at the base of the brain where a smaller tube was then sent from there directly into the AVM. Once inside the AVM a medical "glue" was dispensed into the ends of the ruptured arteries in order to seal them shut in preparation for the surgery tomorrow. The embolization lasts several weeks, so if for some reason the surgery is postponed, he will not have to have this re-done. After the procedure he was alert and said he had a pain level of 7. We were originally told he wouldn't go back to ICU until after the main surgery on Tuesday, but he was sent back to ICU after the procedure and will now be in ICU for probably most of this week, if not more.

Right now I am in the room with him and he is sleeping ... but the past couple hours have been a little rough as his heartrate is accelerated, his blood pressure is extremely high and his fever has returned and continues to rise ... it was at 100.4 when I arrived and is now at 102.6 ... they are ordering a cooling blanket to try and bring the fever down and will be giving him some tylenol in a few minutes. I have also been wiping down his head, shoulders and neck with a cool rag. Right after the procedure his pain level was a 7 and he was given some pain meds, but just a bit ago he was complaining about great pain in his head saying he was at an 9 or 10 so they gave him another shot of pain meds and now he is completely out ... snoring and all. They are bringing him another dose of blood pressure medication now as well (this will be the 4th dose) as his blood pressure isn't going down and is currently 206/72 ... he will have medicine every hour until it gets down to normal (or at least below 150).

The surgeon just came in and said he is still planning on surgery in the morning and will have an MRI later this evening in preparation. His blood has come back without signs of infection and he said it is common for brain injury patients to spike fevers without signs of an infection. He also said that he has him scheduled for the 3rd surgery to replace the portion of his skull removed during the first surgery for the end of next week so long as all the swelling is down and he doesn't have a fever. The surgery is scheduled for 7:15am and he will be taken at 6:30am for pre-op. The surgery tomorrow will remove another portion of his skull to replace the surgery but it will immediately be placed back in place at the end of the surgery. He also said that the embolization that was done today showed an aneurysm going from the brain into the AVM and that was successfuly blocked with the embolization. The surgeon said that the surgery could be as long as 6 hours tomorrow but estimates that it will probably be about 3-4 ... so not really looking forward to that immensely long wait tomorrow. For those of you that pray, specific prayers for Dr. Schievink and his team during the surgery, for Brandon's vitals to remain stable, and for comfort for all of us anxiously waiting during the surgery would be greatly appreciated.

~Melissa
